Glass artwork, from artists like you, is central to the Museum’s identity. We use glass art throughout the Museum, and in our outreach programs as well, to inspire dialogue around key issues relating to liberty and the power of self-expression made possible only through freedom. That is why an auction devoted to glass art is such an essential source of our funding. 100% of the proceeds go directly to our education initiatives, including our Young Heroes Outreach program, which teaches young people throughout our region that they have the ability to make a difference in their school, their community, and beyond.

The entire DONATION AGREEMENT can be completed online below. There are five sections in the form. Here is what you will be asked to provide. We suggest you have everything ready to upload before you begin.
2. Terms of Agreement — The terms are included directly on the online form or you can print this pdf. You will be asked if you accept the terms as you complete the form.
3. Bio — You will be asked to upload a bio of 125 words max in MS Word format for our Auction catalog.
4. IRS W-9 Form — Click here for printable form. If you are a US resident who chooses to receive funds from the sale of your artwork, you will be asked to upload your signed W-9 form.
5. Image of donation for the Auction catalog
(300 dpi jpeg, min. 5”x7”) — You will be asked to upload a photo of your donation, or you may request that NLM photographs your work for you.
