Health Care Heroes Nomination Instructions

Who is eligible to participate?

  1. The Healthcare Heroes Awards recognize individuals age 18 years and older who work in the healthcare and pharmaceutical industries in the United States and around the world.
  2. Nominators and nominees must be able to read, write, and comprehend the English language, which is the language of all program materials.
  3. Employees of the National Liberty Museum, Teva Pharmaceuticals, and members of the immediate family of any such persons are not eligible to participate and win.

Awards Criteria

  1. Category #1 – Hero Researcher or Practitioner in the field of Cancer
    1. The nominee’s actions and achievements have propelled forward the research and/or patient health and treatment in the respective category (Cancer, Crohn’s disease). The nominee has demonstrated innovation, entrepreneurial brilliance, and impact.
    2. The nominee’s professional accomplishments demonstrate profound and significant service to the nominee’s respective field.
    3. The nominee has earned the respect and admiration of his or her professional colleagues, patients (if applicable) and greater community. The nominee is widely considered to be an inspirational role model.
    4. The nominee’s actions demonstrate selfless service to others. The nominee’s actions consistently represent the positive qualities of character celebrated by the National Liberty Museum, for example: integrity, empathy, curiosity, creativity, passion, perseverance, innovation, and courage.
  2. Category #2 – Hero Researcher or Practitioner in the field of Crohn’s Disease
    1. The nominee’s actions and achievements have propelled forward the research and/or patient health and treatment in the respective category (Cancer, Crohn’s disease). The nominee has demonstrated innovation, entrepreneurial brilliance, and impact.
    2. The nominee’s professional accomplishments demonstrate profound and significant service to the nominee’s respective field.
    3. The nominee has earned the respect and admiration of his or her professional colleagues, patients (if applicable) and greater community. The nominee is widely considered to be an inspirational role model.
    4. The nominee’s actions demonstrate selfless service to others. The nominee’s actions consistently represent the positive qualities of character celebrated by the National Liberty Museum, for example: integrity, empathy, curiosity, creativity, passion, perseverance, innovation, and courage.
  3. Category #3 – Community Hero Award
    1. The nominee’s actions and achievements have made a profound and transformative impact on the health and well-being of others, on a local or global scale.
    2. The nominee’s accomplishments in healthcare research and/or treatment demonstrate profound and significant service to others, particularly in underserved communities. The nominee has demonstrated innovation, entrepreneurial brilliance, and impact.
    3. The nominee has earned the respect and admiration of his or her professional colleagues, patients (if applicable), and the greater community. The nominee is widely considered to be an inspirational role model.
    4. The nominee’s actions demonstrate selfless service to others. The nominee’s actions consistently represent the positive qualities of character celebrated by the National Liberty Museum, for example: integrity, empathy, curiosity, creativity, passion, perseverance, innovation, and courage.
  4. Category #4 – Healthcare Hero of Israel
    1. The nominee is a national and/or permanent resident of Israel.
    2. The nominee’s life work in Israel has resulted in breakthroughs in healthcare research, technology, and treatment. The nominee has demonstrated innovation, entrepreneurial brilliance, and impact.
    3. The nominee has earned the respect and admiration of his or her professional colleagues, patients (if applicable), and the greater community in Israel, and beyond. The nominee is widely considered to be an inspirational role model.
    4. The nominee’s actions demonstrate selfless service to others. The nominee’s actions consistently represent the positive qualities of character celebrated by the National Liberty Museum, for example: integrity, empathy, curiosity, creativity, passion, perseverance, innovation, and courage.

Selection Process

The NLM will conduct the initial intake and screening of the nomination submissions, applying the award criteria of the respective Award Category. The nominations will be rated and reduced to a list of finalists. All decisions made are final.

Notification

The NLM will notify the nominators who submitted the four winning awardees. NLM will send letters to the four winning awardees. The winning awardees will be announced on the NLM website.

NLM's Summer Pop-Up Event, August 13-20

Beat the heat and join the National Liberty Museum for our Summer Pop-Up Event! For an entire week, you are invited to browse our silent auction and bid on more than 20 pieces of beautiful glass artworks from the NLM’s collection. Then, the Pop-Up Event week concludes with a live auction during our Cocktails with a Cause program on August 20 at 5 p.m.

Young heroes demonstrate their leadership at a virtual Civics Fair!

At the end of each school year, the NLM's Young Heroes Outreach Program, which brings the Museum’s civic leadership curriculum directly into area schools, holds a grand celebration to showcase the Social Action Projects created by all of the participating schools. This year the celebration was a VIRTUAL event and their projects are available for viewing on this website!
